Entry Forms Released - 2019 National Senior and Youth Championships

The Individual Entry Form and Preliminary Entry Form for Team Events have been released for the 2019 National Senior and Youth Championships, to be held in Latrobe Churchill, Victoria, from 8-13 April 2019.

INDIVIDUAL ENTRY FORM - Instructions
INDIVIDUAL ATHLETES TO COMPLETE THE FORM, STATE/TERRITORY ASSOCIATIONS MUST ENDORSE EACH FORM
- Read and understand the Event Info tab.
- Fill out the Accommodation and Transport tab. Note that TTA has organised heavily discounted accommodation and bus transfers. Both are allocated on a first come, first serve basis. Get in quick!
- Fill out the Individual Events tab.
- Email the completed form to your designated State/Territory Association contact person.
- State/Territory Association to email the completed form to TTA at nationals@tabletennis.org.au.
ALL INDIVIDUAL ENTRY FORMS MUST BE SUBMITTED BY NO LATER THAN 11 MARCH 2019.

PRELIMINARY ENTRY FORM FOR TEAM EVENTS - Instructions
STATE/TERRITORY ASSOCIATION TO COMPLETE THE FORM
- Fill out all information on the form.
- State/Territory Association to email the completed form to TTA at nationals@tabletennis.org.au.
ALL PRELIMINARY ENTRY FORMS FOR TEAM EVENTS MUST BE SUBMITTED BY NO LATER THAN 14 JANUARY 2019.
